Summary
An investigation examines damage to automated pipe-racking and top-drive systems during drill-pipe tripping. Lowering the top drive while pipe remained connected bowed the stand, damaged components and released a guide arm onto the rig floor. Findings identify poor communication, failure to follow the JSA and reduced task focus before crew change.
