Summary
An investigation examines a facial and head injury during pipe tripping on Hercules 300. An air-hoist cable became entangled with a kelly hose safety clamp, loading an anchor post that broke. Findings identify the driller’s blind spot, absent cable-clearance checks and a generic job safety analysis that omitted critical hazards.
