Summary
BSEE investigated a casing connection failure aboard Noble Stanley Lafosse during drilling at GC 478. Seventy-two joints fell partly into the hole and onto the seabed, damaging subsea infrastructure. Findings identified cross-threading, missed connection verification and unclear anti-rotation key installation sequencing. No injuries or environmental impacts were reported.
