Summary
Investigation of a crane boom failure while transferring a 16,000-pound generator from a vessel to an offshore platform. The generator remained attached but became partly submerged. Findings identify operation outside crane limits and shortcomings in pre-use checks and task preparation. Measures address hazard awareness, inspection documentation and operator retraining.
