Summary
Investigation of an unintended lifeboat launch during periodic davit checks on Mærsk Giant at Yme. An incorrectly adjusted manual brake handle reduced braking effectiveness. The empty boat was lost, with nobody injured. Findings examine dynamic brake testing, maintenance documentation, equipment-specific competence, contractor oversight and evacuation procedures.
