Summary
Investigation of a high-pressure riser dropped during permanent well plugging at Jotun B. The riser fell about eight metres onto the wellhead without injuring personnel. Findings address lifting-accessory locking failure, absent secondary retention, uncertified equipment, deficient training, maintenance responsibilities and failures to identify dropped-load risk.
