Summary
BSEE investigated a dropped slick riser joint aboard Noble Don Taylor at Green Canyon Block 282. No injuries were reported, although a worker was nearly struck. Findings identified improper seating of the hydraulic riser running tool and an unengaged secondary manual lock. Subsequent procedural changes introduced a designated locking person and secondary verifier.
