Summary

BSEE investigates a cleanout assembly dropped approximately 40 feet onto a pipe skate aboard the Diamond Ocean BlackHornet drillship. Incorrect elevator inserts, omitted pre-use checks and inadequate verification are examined, including differences between pipe upset types. A floor hand was in the red zone; no injuries occurred.
