Summary
BSEE investigates drill line dropped during crane-assisted coiling on A-Genesis Spar. The line slipped through a nylon strap and fell approximately 80 feet to the catwalk; no injuries were reported. Findings address inadequate rigging, incomplete job safety analysis, absent supervision and procedures, and personnel beneath the suspended load.
