Summary
Investigation of a fire on the unmanned HI A-287 platform. Suspected vibration-related tubing fitting failure released condensate, glycol and gas onto a reboiler. The report describes renewed burning, valve failures and damaged extinguisher hoses, and recommends an alert advocating hard piping and additional bracing.
