Summary
Investigation of a platform fire involving a gas-driven oil pipeline pump. Excessive vibration and an unsuitable pipe nipple contributed to failure, releasing oil onto the engine exhaust. The report examines prior leaks, inadequate exhaust insulation, firefighting and subsequent pumping-system modifications. No injuries were reported.
