Summary
Investigation of a service loop dropped during well tripping ahead of Hurricane Ida. The loop snagged beneath a frac hose gooseneck and was pulled from the top drive. Findings identify unrecognised rig-motion effects and ineffective monitoring; corrective actions address catch points, loop bundling, dedicated observation and weather-specific risk assessment.
