Summary
Investigation of a dropped drill-pipe joint aboard Discoverer India during drilling operations. The pipe became trapped beneath a utility cart during conveyor loading, then fell and damaged the deck and handrails. Barricading prevented personnel injuries. Findings address risk assessment, cart-positioning procedures, staffing changes and crew handover.
