Summary
Investigates two tubular stands with swell packers escaping the fingerboard on Maersk Invincible during well completion. Wind-driven movement opened the latches, allowing the stands to fall across the drill floor without injuring personnel. Findings address unsuitable latch design, inadequate assessment of a supplier bulletin, deficient experience transfer and effective cordoning.
