Summary
Investigation of an oil spill from SP 67 A platform on 6 May 2005. Produced sand wore through a centrifugal pump housing and blocked deck drains, allowing oil to escape overboard. The report records revised spill estimates, platform shut-in, ultrasonic testing and proposed monitoring of sand production using a shake-out flask.
