Summary
BSEE investigates an oil discharge from the unmanned WD 32 E production platform. A stuck transfer-pump valve allowed oil to accumulate in the flotation cell, while incorrectly located shutdown devices failed to detect the high oil level. The report records pollution notifications, enforcement findings and subsequent relocation and testing of the high-level shutdown device.
