Summary
An investigation records a choke-line disconnection during mud flushing on Development Driller II. An incorrectly installed gooseneck connection parted despite repeated pressure tests, releasing a calculated 97 barrels of synthetic oil-based mud overboard. Systems were secured, the gooseneck replaced and a safety line fitted to prevent further overboard discharge.
