Investigation of a crane boom collapse on the Diamond Ocean Saratoga following replacement of a high-angle kick-out valve. Reversed hydraulic flow trapped fluid, locking the controls and disabling hydraulic shutdown. No injuries or pollution occurred. Contractor changes address installation documentation, limit-switch testing and engine emergency shutdown.
