Summary
Investigation of crane boom damage during wireline rig-down at the WD 32-E platform. A falling clipboard obstructed the boom lever, while malfunctioning boom kick-outs allowed further movement. Findings address cab housekeeping, maintenance and pre-use inspection failures. No injuries occurred; subsequent actions included boom replacement and changes to inspection forms and clipboard storage.
