Summary
BSEE investigates a release of an estimated 142 barrels of synthetic oil-based mud from Noble Jim Day into the Gulf of Mexico during mud-pit cleaning. Valve misalignment was linked to communication, labelling and alignment knowledge deficiencies. Findings also address inaccurate drawings, missing lockout/tagout records, environmental hazard analysis and pit-volume alarm response.
