Summary
BSEE investigates a well-control loss during plug and abandonment at West Delta 90 B. An estimated 20 barrels of well fluids escaped, followed by uncontrolled gas and evacuation. Findings identify a spooling-head breach, tubing communication and deficient use of historical well information; the precise origin of the tubing failure remains unresolved.
