Summary
The PSA investigates a dropped side plate during derrick skidding on Gyda. Wear overloaded mounting bolts, releasing the 480-kilogram plate; an operator fell while avoiding it. Findings address maintenance of grippers and skid beams, control-panel positioning, deficient procedures and training, and missed learning from a similar incident.
