Summary
Investigation of a displaced drill-pipe stand recovered on the EW 834 platform. Applying hoist tension moved the stand laterally, trapping a floorman’s foot against rotary bushings and causing fractures. Findings identify inadequate recovery planning, hazard reassessment, supervisory oversight, procedure implementation and controls separating personnel from the load’s movement path.
