Summary

Investigation of a fractured little finger sustained while assembling a BOP test assembly and latching elevators. Rig movement rotated the assembly, trapping the finger between a pump-in sub and bales. Findings address hand placement and equipment positioning, recommending stop-work intervention and updating and discussing the job safety analysis when conditions change.
