Summary
A floorhand suffered severe injuries to both hands while the crew attempted to free a jammed drill-pipe slip insert carrier. A casing wear bushing fell approximately 1.0 to 1.5 feet. The investigation identified poor hand placement, failure to stop and reassess changing conditions, and the absence of a written job safety analysis.
