Summary

Investigation of crude oil pollution following LACT meter proving at the Virgo platform. Open needle valves, an improperly sealed prover access door and a leaking isolation valve were identified. Two barrels went overboard. Recurrence measures include permit-to-work coverage, a revised job safety analysis and verification of valve isolation.
