Summary
BSEE investigates an offshore oil discharge during draining of produced water from a wet oil tank. Findings identify an unattended valve, unsuitable drainage routing and a sand-blocked micro-filter exposing piping leaks. The report records ultrasonic inspection, planned repairs and valve blinding, while finding no evidence that reports were falsified.
