Summary
Investigation of a hot-work fire at Thunder Hawk Platform during torch removal of welds inside a pressurised welding enclosure. Slag passed beneath the enclosure wall and ignited its Velcro portion. Findings address enclosure installation, unavailable secondary firefighting equipment, inappropriate extinguisher use and delayed notification of platform operators.
