Summary
An MMS investigation examines a fire during well abandonment when acetylene cutting ignited a tarpaulin and hydrocarbons in a nearby stock tank. Findings identify inadequate separation of flammables before hot-work authorisation and absence of a welding JSA. Water and foam extinguished the fire in approximately 30 minutes, without injuries or pollution.
