Summary
Investigation of a well-control incident during workover and completion on Platform Gail. Removal of a wellhead lockdown pin bypassed the BOP system, while discontinued fluid pumping allowed an underbalanced condition. The report describes hydrocarbon release, hydrogen sulphide readings, evacuation and an approximately three-gallon oil spill, with recommendations on well-control practices and training.
