Summary
An investigation examines severe finger injuries during riser installation on Eugene Island 354 D platform. A contract diver inadvertently increased air-tugger cable tension after using oppositely configured controls. The report identifies a pinch point, missed pre-job safety review and inadequate supervision. The tuggers tensioned piping rather than lifted loads.
