Summary
BSEE investigates an uncontrolled personnel-basket descent during transfer from Cantium’s offshore platform, injuring a worker who remained inside. Testing examined the auxiliary winch, worn sheave assembly and anti-two-block system without establishing a reproducible root cause. The report identifies unresolved maintenance deficiencies, oversight weaknesses and delayed evacuation, recommending improved emergency planning.
