Summary
Investigation of a crane kingpost separation during basket lifting on the Mars platform. Metallurgical analysis identified progressive fatigue, most likely initiated by Hurricane Katrina impact damage, while discounting material and fabrication defects. The report examines inspection limitations, operational loading and the evacuation, stabilisation and removal response. No injuries or pollution occurred.
