Summary
Investigation of a casing joint that separated from a lift nubbin at approximately 40 feet above the rig floor during liner running, injuring a crew member. Thread inspection indicated improper make-up of the connection, despite a pre-job JSA requiring proper installation. The injured worker was evacuated for hospital treatment.
