Summary
Investigation of a casing-handling accident on Hercules 205. Elevators lodged in the derrick A-frame, and continued pulling caused sling and swivel failure. Falling casing struck tongs, displacing the operator from his platform and breaking his neck. The pre-job analysis identified the snagging hazard, but nobody was assigned to watch for it.
