Summary
An investigation examines a riser slip-joint seal failure during cement-tank fluffing. Reduced rig air pressure caused 20.8 barrels of synthetic-based mud to spill into the moon pool. Compressor alignment, control settings and the absence of a check valve contributed; restoring air pressure stopped the leak, and no sheen was observed around the ship.
