Summary
A subsea engineer on the West Auriga drillship suffered facial injuries when pressurised BOP fluid escaped during SPM valve O-ring replacement. The investigation identified residual pressure, a gauge stuck at zero, a leaking relief valve, closed isolation valves, inadequate assessment of the added task and awkward valve positioning.
