Summary
Investigates injuries to two technicians during casing-valve maintenance on Main Pass 311 B. Unexpected casing pressure ruptured gas-lift tubing, knocking both workers down. Findings address misleading SCADA readings, unmonitored casing pressure, omitted diagnostics and pre-job checks, and discrepancies between installed equipment and approved drawings. Subsequent changes introduced direct pressure readings.
