Summary
An investigation into serious injuries during crane dismantling on South Marsh 136 B platform. A worker released a bridle pin before gantry tension was relieved, then fell approximately 19.5 feet with his lanyard attached to the bridle. Findings address unsuitable anchorage, unsecured bridle support, inadequate procedures and deficient supervision.
