Summary

A BSEE investigation examines an injury during pipeline removal on an unmanned platform. A tensioned pipeline moved approximately three to four feet after cutting, striking a worker and throwing him through an open hole into the Gulf. Findings address hazard reassessment, supervision, missing mechanical bracing and failure to stop work.
