Summary
BSEE investigates a nitrogen-cylinder rupture aboard Helix Q5000 during workover operations. Three personnel sustained minor shrapnel injuries. The report identifies corrosion-related wall loss, inadequate coating protection, incomplete visual inspections and poor rack drainage. It describes muster and cylinder venting, alongside planned nitrogen generation, protective barriers and changes to vendor selection and inspection.
