Investigation of lost well control during temporary abandonment of the B002 well at Ship Shoal 225. Pulling a plug without checking pressure beneath it preceded hydrocarbon escape through leaking tubing and casings. The report examines deficient records review, testing, procedure changes, site-specific hazard analysis and supervision, and recommends stronger abandonment controls.
