Summary
An investigation examines an offshore discharge during liner cementing. Misreading an isolation-ball pressure increase led to early termination of cement displacement and circulation of cement to surface. Deliberate disposal of contaminated fluid released approximately 204 barrels of synthetic-based mud. Recommendations address contingency arrangements and clarification of interface discharge limits and reporting.
