Summary
BSEE investigates the Pacific Sharav’s discharge of 29 barrels of synthetic oil-based mud into the Gulf of Mexico during deck-drain water transfer. Misaligned low-pressure header valves connected the active mud system to the overboard line. Findings address the absent Mud Engineer, procedural compliance and subsequent changes to valve-alignment verification.
