Summary
An investigation records an overboard release during drilling-mud transfer on 27 June 2007. A master dump valve left open after cleaning allowed 33 barrels of synthetic mud, containing approximately eight barrels of oil, to escape. Recommendations address valve closure, employee training, shift handover and revision of the job safety analysis.
