Summary
Investigation of a synthetic-based mud discharge during casing operations. A leaking packing seal allowed an air-operated diverter valve to open, releasing approximately 19 barrels from the trip tank. Findings address absent preventive maintenance and idle-valve monitoring, with subsequent valve replacement scheduling, evaluation of an alert system and revised pollution notification procedures.
