Summary
Investigation of a drillship near miss in which inadvertently opened elevators released drill pipe onto a pipe skate, with no reported injuries. Findings examine joystick selection, inactive software and mechanical interlocks, insufficient commissioning and training. Follow-up included manufacturer system tuning, further crew training and consideration of joystick redesign.
