Summary
An investigation of an offshore production incident in which approximately two barrels of oil entered the sea. Sight-glass maintenance bypassed a high-level safety function; valve leakage, open drains and blocked sump piping contributed to releases during platform blowdown. Corrective measures included bridle separation, valve servicing, drain rerouting and further operator training.
