Summary
Investigation of a pipeline valve actuator ejection at Brazos A133 Platform A. The actuator travelled approximately 25 feet upwards and struck a deck support beam, without reported injuries, pollution or gas release. Investigators suspected pressure entering the actuator’s exhaust/vent side but could not reproduce the event under shut-in conditions.
