Summary
An investigation records a head injury during plug and abandonment work aboard Liftboat Juan. Failure of the crane’s anti-two-blocking system caused the auxiliary line to part, dropping a lubricator and ball onto the deck. The lubricator struck the worker’s hard hat; the laceration required eight staples and restricted duty followed.
